Getting Started
A little paperwork, and then the real work begins. Before your first session, I'll ask you to complete two brief forms — a short questionnaire and a services agreement. Both take just a few minutes, and everything you share stays between us.
Answer a few questions
Fill out the brief questionnaire below so I can learn a little about you before we meet.
Review and sign the agreement
Download the services agreement, read it at your own pace, and sign it.
Send it back
Email a copy or photo of the signed agreement to me. That’s it — you’re ready for your first session.
The Therapy Agreement
This agreement covers the practical side of working together — session fees and payment, scheduling and cancellations, how teletherapy sessions work, and my confidentiality practices, including the few situations where the law requires disclosure. I ask every client to read and sign it before our first session so there are no surprises, and we can give our full attention to what actually matters: you.
If anything raises a question, reach out before we meet or bring it to our first session — I'm glad to walk through any of it.
About the forms
-
You don't need a printer.
On iPhone: Open the PDF, tap the Markup icon (the pen tip), tap +, and choose Signature. Sign with your finger, place it on the signature line, then share the saved PDF to my email.
On Android: Open the PDF in the free Adobe Acrobat app, tap the pencil icon, choose Fill & Sign, and tap the signature tool to sign with your finger. Then share it to my email.

Yes. Confidentiality is the foundation of therapy, and it begins before your first session. The questionnaire asks only basic contact and background questions, and the services agreement explains my confidentiality practices in full detail — including the few, rare situations where the law requires disclosure.
-
Ideally before your first session, so we can spend our time together on you rather than on paperwork. If that's not possible, don't worry — just bring your questions and we'll sort out the forms together.

Reach out. Technology should never be the thing standing between you and getting started. Email me at kristenfong.mft@gmail.com and I will help, or we can simply handle it another way.
Questions before your first session?
Reaching out is often the hardest step. I'd be glad to hear from you.
